Patient Success Story – Robert
Robert, who was experiencing homelessness when first referred from Baptist Beaches three years ago, came into our Beaches Health & Wellness program with an A1c at 11.1; his latest result was 5.7! Today he is working and remains housed, thanks to ongoing patient health education, coaching, and support. He is even seeing the specialists at the Hangar Clinic for custom shoes with linings to support patients with diabetes to make the long hours he works in food service possible without further damage to his feet.
Since COVID safer at home measures began, RN Case Manager Tina touches base with Robert via phone every two weeks. He maintains a daily food diary, stays on his medications, and follows Tina’s advice for referrals to podiatry, primary care, lab appointments, and more. Her care has helped support his independence and productivity.
Robert’s case is the definition of patient-centered continuity of care. The care he has received over the past three years is a direct reflection of the assistance WeCareJax works to provide for every patient.
