Patient Success Story . – John
John is a 21-year-old African American who had a job in a restaurant and was renting an apartment in Jacksonville Beach when he started having trouble breathing. Having no insurance, John went to the Emergency Room at Baptist Beaches Hospital. He was admitted to the hospital for a rather long stay. Because he would need follow-up care after discharge and the fact that he had no insurance, John was referred to Tina Crooks RN/Case Manager for WeCare.
We Care Jacksonville helps low-income, uninsured residents of Duval County who need medical specialty care. WeCare is made up of medical specialists, hospitals, laboratories and diagnostic centers who donate their services to eligible patients.
Tina helped John receive follow-up primary care at I.M. Sulzbacher Beaches Community Healthcare Clinic. She helped him by scheduling appointments with a pulmonologist who ordered CT scans and medicine to treat his condition. All of these services were donated to John through WeCare’s network of volunteers.
With Tina’s help, John signed up for the Prescription Assistance Program with the pharmaceutical company so that he would continue to receive the medications he needed.
Because he was sick for so long, John lost his job and was served an eviction notice at his apartment. Tina brought John to BEAM (Beaches Emergency Assistance Ministry) on Jacksonville Beach where he showed them his lease and they gave him an emergency advance of one month’s rent. John also received food from BEAM.
John regained his strength and got a job with health insurance. Through a coordination of community services, John was able to stay in his apartment and is now able to pay his bills and see a doctor with his own insurance.
We are happy to say that John no longer requires WeCareJax’s services. This makes him a success!
