How do I qualify for WeCareJax?

To qualify for participation in WeCareJax patients must meet the following eligibility criteria:

  • Patient must be a resident of Duval County
  • Patient must be uninsured
  • The combined household income may not exceed 300% of the federal poverty guidelines (see the table below)
  • Patient must be referred by a physician or clinician from one of the health safety net clinics or a hospital or emergency department. Once a referral is received, only the WeCareJax coordinator may schedule an initial appointment.

Note: You may be experiencing homelessness or living in Duval County without legal residency status and still qualify for care.

2025 Federal Poverty Guidelines

For each additional person over the family size of 8, add $1,374

The timing of the appointment is based on the specialty physician availability but we work with providers to obtain the next available appointment.

WeCareJax can assist with most medical specialties and can include lab tests, imaging and diagnostic testing.  All services are based on provider availability.

There is not a set enrollment period as treatment plans differ significantly; however, eligibility is reviewed every year at a minimum. Meeting financial eligibility and living in Duval County is a requirement to continue care.

Please bring the following documents with you to your appointment:

  • Official Photo ID
  • One of the following:
    • Copy of pay stubs for the past four weeks
    • Most recent tax return
    • Letter of employment verification with hourly rate from your employer.
    • Letter of support from a shelter or significant other, if you are unemployed
